Packages containing UN3158 Gas, refrigerated liquid, n.o.s.* must affix Div. 2.2 hazard labels and RCL handling label. Is it necessary to show both Cargo IMP code of RNG and RCL on the NOTOC? (31 Aug 10)

The NOTOC form is an airline form and is not altogether common. ICAO only lists the items to be included on the NOTOC as stated in DGR 9.5.1.1.1 and DGR 9.5.1.1.3. The form itself is left to the airline to design. The method of entry is also left to the airline. The normal practice is to show all applicable 3-letter Cargo IMP code on the NOTOC, however, it is up to the airline to Dec de their own procedure.
